Why Person-Centred approaches are at the heart of what we do.




When it comes to providing services for the individual, a 'one-size fits all' approach just doesn't cut it. At Happy Hearts Home Help we want to learn about each client. Taking this approach means that we can understand their needs, goals and aspirations and how we can help them to reach their full potential.


We use tools such as the One Page Profile: which allows the client to communicate what people appreciate about them, what is important to them, and how they would like to supported. Good Day/Bad Day: which helps us to understand the likes, dislikes and triggers of each client with real and specific examples, and Learning Logs which help us to understand behavioural patterns so that we can help to anticipate the best approach for support and activities when it comes to client satisfaction.


Using these tools help us to tailor our support and delivery to promote the client's wellbeing and meet their holistic needs. The knowledge and understanding of our client grows over time, adapts with change, and puts the individual in control. For a fully tailored and personalised experience, which enriches and enhances their life.
